Date of Posting: April 12th, 2024 Date of Closing: April 19th, 2024 Start Date: As soon as possible Title: Team Lead Assist, Community Care Reports To: Registered Nurse/Director of Nursing Position Summary: Working within Community Care in an interdisciplinary, team orientated, collaborative practice environment, the Community Care Team Leader is responsible for the day- to-day management of the community care floor. The Team Leader supervises and collaborates with Resident Care Workers (RCW), Licensed Practical Nurses (LPNs) and other health professionals and / or assumes direct care provision for the resident’s required care. The primary purpose of this position is to supervise and manage the provision of current best practice, highest quality, holistic, person centered and family focused care delivery. The Team Lead Assist is responsible to: Model, Promote and Supervise the Professionalism of the Staff: •practice within appropriate legal parameters •verify resident consent before initiating care •advocate and protect the rights of the resident •assume responsibility for one’s own actions at all times •verify orders and procedures •take steps to maintainoptimal personal health and safe work environment •perform within role parameters, meets and maintains expected level of competency •obligated to report to appropriate authority all incidents of unsafe practice or professional misconduct •seek assistance and guidance when the care required exceeds personal training •incorporate, model and encourage the behaviour of others to reflect the Person-Centered Care Model •promote cooperation and teamwork •be a team leader and manage a team of staff •respect the spiritual beliefs, dignity and rights to privacy of every resident •consult with other health professionals Supervise, Monitor and Perform Resident Care Assessments: •monitor temperature, pulse, respirations, blood pressure, blood glucose and respond to diabetic symptoms and reactions •assess resident’s appearance, resident’s health condition, skin changes, changes in elimination, changes in circulation and oxygenation, acute symptoms of respiratory distress, circulatory distress, gastric distress, inflammation, shock, fluid and electrolyte imbalance, intestinal distress, urinary distress, cerebral and neurological distress, infection and pain Demonstrate and Supervise the Utilization of Therapeutic Communication: •provide appropriate information to and receive information from the resident, their family and all members of the health care team •communicate with the resident to facilitate their understanding •encourage the resident’s participation in planning and implementing their care plan •encourage the resident to assume responsibility for their own health •establish therapeutic communication with the resident •apply charting principles •foster good communications within and amongst departments and services •offer a welcoming environment for residents, families and all staff •utilize conflict resolution and mediation skills to resolve issues between residents, their families and staff •prepare and deliver shift report to oncoming team •participate in physician/ healthcare professional rounds •communicate in a respectful, courteous and helpful manner with everyone Monitor the Utilization of Devices and Equipment: •use equipment that helps promote healing, prevention of deformities or minimizes the effects of iimmobility •use resident lifts •care for residents who require assistive devices, including assessment and maintenance of equipment (Need for maintenance is reported to the appropriate resource) •make beds •operate equipment safely and appropriately •identify equipment needs •monitor the maintenance, use and cleaning of equipment •evaluate client equipment needs and communicate same to the appropriate authority •apply bandages, triangular slings, cervical collars, splints, TED stockings, tensors and braces •teach resident / family how to use and operate equipment that help promote healing, prevent deformities or minimize the effects of immobility Monitor Personal Hygiene and Daily Living Support/ Moving in, Discharge and Transfer: •assist with personal care (bathing, oral care, nail care, hair care and perianal care) •assist with eye care •assist with dressing and undressing orientate resident to household/ unit and room •care for and secure resident valuables •lead discharge planning •provide and reinforce discharge orders with the resident and / or family •direct arrangements for transfer of the resident Monitor and Provide and Evaluate Protection and Comfort: •assist and maintain a safe resident environment •verify resident identification •ensure resident accessibility to the call bell system •promote relaxation and sleep •assist resident to meet emotional, psychological and mental needs •use safety devices including restraints in accordance with least restraint policy •apply principles of good body mechanics •ensure the removal from resident’s room and dispose of soiled incontinent pads, soiled linens, etc. •ensure the stock and availability of adequate linens for use with residents •manage responsive behaviours
Monitor and Provide Safety and Emergency Care/ Provide Care for Dying and Death: •perform basic first aid measures (within scope) •perform basic emergency procedures including CPR, anti-choking manoeuver •practice principles of safety and accident prevention i.e WHMIS, Falls Prevention •practice standard precautions i.e. Infection Control •review and conduct regular Fire Safety, Evacuation and Emergency Planning Drills •participate in meeting the needs of the dying resident and their family •care for the deceased resident, the family and the resident’s personal possession Monitor and Assist with Mobility: •encourage resident to practice active exercise and range of motion •organize and encourage recreational activities •provide assistance when positioning a resident •assist and encourage resident mobility •assist with the use of ambulatory devices •collaborate with other health team members to deliver special exercise programs
Administration: •verify medication orders •research ordered medication prior to administering it •prepare and administer medications by various routes •demonstrateappropriate use of the medication cart •monitor, report and record resident’s response to medication administration •manage medication stock and records •ensure resident medications are ordered and reassessed on a regular basis manage narcotics stock •Dr’s rounds and processing Dr’s orders •Assess staff medication competencies
Leadership and Management: •Interview, hire, train new staff •Have documented employee discussions, have employment disciplinary conversations •Manages and leads a team of employees. Communicates company goals, safety practices, and deadlines to team. Motivates team members and assesses performance. Provides help to management, including hiring and training, and keeps management updated on team performance. Communicates concerns and policies among management and team members. •Supports managers and performs management duties when manager is absent or out of office •Provides encouragement to team members, including communicating team goals and identifying areas for new training or skill checks •Answers team member questions, helps with team member problems, and oversees team member work for quality and guideline compliance •Communicates deadlines and care goals to team members •Conducts team meetings to update members on best practices and continuing expectations •Ensures company brand materials and physical working spaces meet and exceed company presentation standards •Provides quality customer service, including interacting with residents and families, answering enquiries, and effectively handling complaints
Requirements: Diploma from an approved school with a health care related course. Preference will also be given to Primary Care Paramedics. Experience at a supervisory level. Considerable experience in a long-term care setting is an asset. Excellent communications, interpersonal, problem-solving skills and judgement. Working Conditions: Work days are generally 4-5 days per week, with shift lengths from 4-12 hours per day. Includes days, evenings, weekends, nights, and holidays. Physically demanding, involving bending and reaching, and heavy lifting of items as well as patients. Must be able to stand for extended periods of time.
Position available: PERM 40% Team Lead-60% RCW Wage Range:$21.35-$22.35
