Job Posting: Community Care Evening Supervisor Closing Date: Until filled Starting Date: As soon as possible The Mount Continuing Care Community is looking for casual a Community Care Evening Supervisor in our long-term care facility in Charlottetown, PEI. This position reports to the team leader, RN or Director of Nursing, and assumes authority, responsibility, and accountability for the delivery of nursing services in our continuing care community.
The successful candidate will possess the following qualifications (please note only candidates with the following qualifications will be considered):
Diploma from an approved school with a health care related course.
Preference will also be given to Primary Care Paramedics and LPN.
Experience at a supervisory level.
Considerable experience in a long-term care setting is an asset.
Excellent communications, interpersonal, problem solving skills and judgement.
A valid criminal record check and vulnerable sector check.
Up to date CPR training.
Up to date immunizations and TB testing.
GPA and TLR training.
Main responsibilities include but are not limited to:
Answer call signals; assist residents to the bathroom; supply and empty bed pans; bathe, dress and groom residents; serve meal trays, feed or assist in feeding of residents and assist residents with menu selection; weigh, lift, turn, and position residents; monitor nutritional intake; supervise residents' exercise routines, set up and provide leisure activities for residents, accompany residents on outside recreational activities and perform other duties related to resident care and comfort.
Assess and respond to the resident’s appearance, health condition, skin changes ,changes in elimination , changes in circulation and oxygenation and assess acute symptoms of respiratory distress, circulatory distress, gastric distress, inflammation, shock, fluid and electrolyte imbalance, intestinal distress, urinary distress, cerebral and neurological distress, infection and pain.
Provide activities for residents, promote person centered care, socialize and engage with residents for activities of daily living. Administer medications in the Community Care neighborhoods.
Take residents' blood pressure, temperature and pulse; report or record fluid intake and output; observe or monitor residents' status and document resident care on charts; administer first aid in emergency situations; collect specimens such as urine, feces or sputum.
Provide appropriate information to and receive information from the resident, their family and all members of the health care team.
Communicate with the resident to facilitate their understanding.
Encourage the resident’s participation in planning and implementing their care plan. As well, encourage resident to assume responsibility for their own health.
Apply charting principles .
Foster good communications within and amongst departments and services.
Prepare and deliver shift report to oncoming team.
Utilize conflict resolution and mediation skills to resolve issues between residents, their families and staff .
Communicate in a respectful, courteous and helpful manner with everyone.
Participate in the move in , transfer and discharge process.
Verify medication order.
Prepare and administer medications by various route.
manage medication stock and records.
ensure resident medications are ordered and reassessed on a regular basis manage narcotics stock.
Participate in Dr’s rounds and processing Dr’s orders.
Manages and leads a team of employees. Communicates company goals, safety practices, and deadlines to team. Motivates team members and assesses performance. Provides help to management, including hiring and training, and keeps management updated on team performance. Communicates concerns and policies among management and team members.
Answers team member questions, helps with team member problems, and oversees team member work for quality and guideline compliance
Follow proper resident transfer policies (TLR).
Make beds and maintain residents’ rooms.
Maintain inventory of supplies.
Clean and maintain all medical equipment.
Ability to handle sensitive information with the highest degree of integrity and confidentiality.

Shifts may range from 3 to 12 hours, with a combination of days, evening, nights, weekends, holidays. The successful applicant must be able to work a combination of shifts, with flexibility.
