In accordance with the philosophy, procedures, and policies of The Mount Continuing Care Community, and under the direction of the Director of Nursing, the Team Lead is responsible for day-to-day management of the Community Care floor. The Team Lead supervises and collaborates with Resident Care Workers (RCWs), Licensed Practical Nurses (LPN) and/or assumes direct care provision for the resident’s required care.
The successful candidate will possess the following qualifications:
Licensed Practical Nurse in good standing with the provincial regulating body.
Diploma from an approved school with a health care related course will be considered.
Experience at a supervisory level is an asset.
Considerable experience in a long-term care setting is an asset.
Main responsibilities include but are not limited to:
Being familiar with and following the Operation and Care Service standards for Community Care and current policies and procedures
Utilize the nursing process as a guide to assess, plan, create goals, evaluate and utilize appropriate interventions for resident care
Collaborate with members of an interdisciplinary health team to plan, implement, coordinate and evaluate patient care in consultation with patients and their families.
Administer medications and treatments as s by a physician or according to established policies and protocols.
Monitor, assess, address, and document any symptoms and reports changes in patients' conditions to the Registered Nurse
Take residents' blood pressure, temperature and pulse, height and weight; observe or monitor residents’ status and document resident care on charts; administer first aid in emergency situations; collect specimens such as urine, feces or sputum; administer suppositories, colonic irrigations and enemas and perform other procedures as directed by nursing and hospital staff.
Coordinate appointments with physicians and other health care providers
Operate or monitor medical apparatus or equipment.
Monitor daily operations of the Community Care Unit
May supervise licensed practical nurses and other nursing staff.
Ensure appropriate resident care is being performed by care staff
Facilitating the resident admission process
Participate in the staff evaluation process
May develop and implement personal care plans on admission of patients.
May teach and counsel patients and their families on health-related issues in collaboration with other health care providers.
Complete the Seniors Assessment Screening Tool (SAST) as necessary
Working Conditions:
Work days are generally 4-5 days per week, with shifts running 4 to 12 hours per day.
75 hours bi-weekly.
Physically demanding, involving bending and reaching, and heavy lifting of items as well as patients.
Must be able to stand for extended periods of time.

$19.40-21.40 per hour, depending on experience.
